RAISING A FORMAL COMPLAINT

Please telephone and speak to a member of the management team prior to raising a formal complaint as we may be able to resolve your concern or issue the same day.

Please find below the Practice complaints leaflet along with formal complaints form. 

If you are raising a complaint on behalf of someone else, you must ensure we receive their written consent to speak to you. Without this unfortunately we are unable to disclose any information regarding the patient.
