We are currently experiencing an extremely high volume of telephone calls from patients, some of which have been abusive or unnecessarily rude towards our Reception Staff, who are answering the phones as quickly as possible throughout the whole day and have been working tirelessly throughout the entire COVID-19 Pandemic.
We will not tolerate abuse towards any of our staff and clinicians, and anyone found to behave this way may face removal from the Practice List.
You can book appointments online every day from 8am using the NHS App which can be downloaded to your smartphone HERE
Type 1 Opt Out (Data Sharing) / National Data Opt Out
The Government has decided it will collect, process, analyse and disseminate your GP data for Planning and Research Direction 2021.
From 1st July 2021, the patient data will be extracted from GP systems on a daily basis; data will be encrypted and pseudonymised.
As this is a statutory obligation, practices are legally obliged to share this data. Click HERE to learn more about what the government says.
You do not need to do anything if you are happy for your data to be shared in this manner.
If you as a patient want to opt out of your data being shared, apart from information used for your direct care, you have the option of registering a Type 1 Opt Out or a National Opt Out or both. To opt out of the National Data Opt Out - Please visit https://www.nhs.uk/your-nhs-data-matters/ or telephone 0300 303 5678 Monday to Friday 9am to 5pm.
If you wish to register your Type 1 Opt Out choice, you will need to print a form, sign it with your preferred option and then send it back to the surgery by post or email it to firstname.lastname@example.org If you do not have access to a computer or printer then you can collect a form from the front lobby of the surgery. You can download the form HERE
You must return the Type 1 Opt Out Form to us by 23rd June 2021 to ensure no data is uploaded from your records.
PLEASE DO NOT TELEPHONE THE SURGERY IF YOU HAVE FURTHER QUESTIONS AS WE WISH TO KEEP THE PHONE LINES OPEN FOR PATIENTS NEEDING DIRECT CARE

Seven Day Access Service
The Seven Day Access Service (for Heywood, Middleton and Rochdale) allows patients to have more flexibility in where, when and how they use local health services. The service helps patients who have difficulty in getting to the doctors during usual opening times by having appointments available outside of these hours.
While patients may not get to see their own doctor, the GP or Nurse they do see will have access to the patients medical record (subject to their consent) and the service can provide all of the same services you can access at your GP surgery.
Bardoc, working in partnership with GP Care, provide the Seven Day Access Service at Four Hubs across the borough (listed below)
